The Community Navigator provides front-line assistance to residents accessing social and community services, helping individuals complete applications, understand options, and connect with needed supports.
Key Responsibilities
Provide direct support to individuals seeking access to social services, housing, and health supports
Assess needs through intake conversations and identify appropriate community resources and referral pathways
Connect individuals with service providers and follow up to support successful navigation
Assist clients with completing applications, forms, and documentation
Maintain accurate, confidential case notes and records
Qualifications
Post-secondary education in Social Services, Public Health, Human Services, Community Development, or similar field
Experience working in social services, case management, or related role
Strong understanding of community resources, social assistance programs, housing supports, and health services
Strong communication, interpersonal, and relationship-building skills
Valid Class G driver's license required; Vulnerable Sector Check required prior to employment
